Robotic Therapy and Transcutaneous Electrical Stimulation in the Painful Shoulder Treatment of Hemiplegic Patients

Abstract

Introduction: Hemiplegic shoulder pain as a consequence of stroke is a common, feared and debilitating complication conditioning a limited scope of functional recovery. It usually appears in 22-23% of survivors and in almost 54% of rehabilitation patients.

Objective: To evaluate the effectiveness of robotic therapy in patients with hemiplegic shoulder pain syndrome.

Methods: A therapeutic intervention (a pilot study) was carried out in 16 stroke survivors who presented hemiplegic painful shoulder syndrome as a complication, in flaccid and spastic stages, to facilitate evaluating the effectiveness of the application of robotic therapy. The variables analyzed were age, sex, affected hemibody, flaccid or spastic stage, time of evolution, degree of pain severity, muscular and joint evaluation, degree of spasticity, therapeutic response, degree of satisfaction. The patients who met the selection criteria were randomly assigned to two groups: robotic therapy and transcutaneous electrical stimulation and they were treated at the Department of Physical Medicine and Rehabilitation of “Dr. Juan Bruno Zayas” General Hospital, in Santiago de Cuba.

Results: Both treatments showed effectiveness in the treatment of painful hemiplegic shoulder. As a result of robotic therapy, patients showed significant pain and spasticity control, increased joint range, and expressed high level of satisfaction, making it a promising technology for such treatments.

Conclusions: The effectiveness of robotic therapy yielded very satisfactory results in patients with hemiplegic shoulder pain syndrome.
