Valor de la fibrosis en la hepatitis crónica activa

Abstract

La hepatitis crónica activa es una enfermedad cuya evolución es poco predecible: puede ser prolongada y estática o evolucionar a la cirrosis hepática en períodos más o menos cortos. El diagnóstico realizado mediante laparoscopia y biopsia hepática, permite definir el grado de fibrosis hística en el momento de realizarse el diagnóstico y relacionar su intensidad con la evolución posterior endoscópica del paciente. Se estudiaron 40 pacientes con hepatitis crónica activa con evolución de más de 2 años después del diagnóstico inicial, seguidos mediante laparoscopia y biopsia. Las biopsias se clasificaron en: sin fibrosis y con fibrosis, ésta a su vez en ligera y moderada, según los criterios utilizados habitualmente. De los 40 pacientes estudiados, 16 no presentaron fibrosis en la biopsia inicial, de los cuales el 55% tuvo una evolución favorable y el 45% una evolución desfavorable. Según el grado de fibrosis, se observó que el 55 % y el 85 % de los que tenían fibrosis ligera y moderada respectivamente al diagnóstico inicial, evolucionaron desfavorablemente.
