Patogenia de la diabetes mellitus no insulinodependiente. Revisión bibliográfica
Abstract
Al igual que la diabetes mellitus insulinodependiente (DMID), los estudios patogénicos de la diabetes mellitus no insulinodependiente (DMNID) han demostrado lo heterogéneo del síndrome. Se ha comprobado que la mayoría de los DMNID son obesos, aunque también los hay no obesos y con una base genética. En los DMNID obesos, se ha descrito sensibililad disminuida del mecanismo glucorreceptor, grados variables de secreción de insulina, lo que incluye la hiperinsulinemia-resistencia insulínica. En los DMNID no obesos, se ha postulado que en el fracaso del glucorreceptor de la célula beta desempeñe algún papel la prostaglandina E. Es planteado también que en la DMNID, el eje entero- insular, glucose-dependent-insulinotropic polypectide (GIP) influye en la insulinosecre- ción. Las consideraciones antes realizadas, evidencian la heterogeneidad de la diabetes y, por ende, lo variable e individual de su tratamiento.Downloads
