Diagnóstico laparoscópico de la hepatitis crónica

Abstract

Se analizan 102 laparoscopias realizadas a pacientes que padecían hepatitis crónica; mediante esta técnica se diagnosticó a 50 de estos enfermos, una hepatitis crónica persistente (HCP), y a 52, una hepatitis crónica agresiva (HCA), diagnóstico presumible que fue corroborado mediante biopsia hepática realizada simultáneamente. Puede establecerse que en las HCP la superficie del hígado suele ser lisa, y la consistencia puede estar normal o aumentada casi por igual. Se plantea que el borde es regular, y es posible observar perihepatitis. No se constata la presencia de circulación colateral. En las HCA la superficie casi nunca es lisa, la consistencia se encuentra prácticamente siempre aumentada, puede existir circulación colateral, así como presencia de nodulos en el borde, y no suele constatarse perihepatitis. En ambos tipos de hepatitis predominó el borde fino y el color rojo pálido. Se concluye que la laparoscopia es un método valioso para el diagnóstico diferencial de estos tipos de hepatitis crónica.
