Enfermedad relacionada con IgG4
Abstract
La enfermedad relacionada con IgG4 se caracteriza por fibrosis inflamatoria acompañada de concentraciones séricas de IgG4 elevadas. Clínicamente puede manifestarse con afectación de un solo o de múltiples órganos. Se ha propuesto que esta enfermedad es secundaria a la interacción entre células Th2 y células B, que resulta en concentraciones elevadas de IgG4 y mediadores inflamatorios lo que resulta en infiltrados linfoplasmocitarios y fibrosis; sin embargo, no es el único mecanismo involucrado. Para el diagnóstico se requiere criterios clínicos, serológicos, histopatológicos e inmunohistoquimicos. Se ha observado buena respuesta al tratamiento esteroide. Se reporta el caso de una mujer de 30 años con derrame pleural a repetición, neumopatia intersticial, alteración de la función renal progresiva y elevación de paratohormona y calcio en sangre. Los estudios revelaron altos niveles séricos de IgG4, biopsia de pulmón y riñón consistente con enfermedad relacionada con IgG4. La paciente fue tratada con 40 mg día de prednisona con excelente respuesta.Downloads
