Severe Lenticular Subluxation in Marfan Syndrome

Introduction: Marfan syndrome is frequently associated with lens ectopia. Standard management involves intraocular lens (IOL) implantation; this requires precise biometry and the availability of specialized IOLs, conditions that may not be feasible in resource-limited settings.
Objective: To report the successful management and visual outcome in a case of severe lenticular subluxation associated with Marfan syndrome.
Case Report: A 36-year-old male patient with Marfan syndrome presented with progressive vision loss. He exhibited severe bilateral lens subluxation (> 270°) and cataracts. Given the inability to perform biometry or obtain IOLs, bilateral intracapsular lens extraction was performed. The patient remained aphakic and was corrected with contact lenses, achieving a final visual acuity of 20/20 in both eyes. Intracapsular lens extraction followed by aphakic correction represents a viable and effective surgical option for complex cases of lens ectopia when modern techniques involving IOL implantation are not accessible. This case underscores the need to develop strategies to improve access to advanced ophthalmological technologies across all regions.
