Pancreatic Panniculitis

We report a case of pancreatic panniculitis in a 66-year-old woman residing in Havana, Cuba, with a history of renal allograft transplantation 21 years prior. Clinical manifestations included generalized abdominal pain, asthenia, adynamia, and fever. Following hospital admission, a diagnosis of acute pancreatitis was established; laboratory reports documented a significant elevation of pancreatic enzymes. The Dermatology Service was consulted to evaluate the patient due to the presence of painful, erythematous nodular lesions on the lower extremities, which appeared 12 days after the onset of pancreatitis.
A skin biopsy confirmed the diagnosis of pancreatic panniculitis, with the pathology report noting lobular panniculitis and necrobiotic adipocytes. The Surgery Service recommended expectant management; follow-up measurements showed a decline in pancreatic enzyme levels, and the skin lesions resolved spontaneously within two months, leaving only residual hyperpigmentation.
This case is notable for its atypical chronological presentation—specifically, the appearance of cutaneous lesions two weeks after the onset of the pancreatic disturbance—as well as the condition's low incidence in the general population.
