Chronic Tophaceous Gout with a Tumor-Like Appearance and Reconstruction of Osteoarticular Damage by Three-Dimensional Computed Tomography

Introduction: Chronic tophaceous gout with a tumor-like appearance represents the final and most disabling stage of gout, in which early diagnosis and treatment are crucial to prevent devastating complications.
Objective: To describe the evolution of a clinical case of complicated gout with systemic involvement in a patient who is not adherent to treatment.
Case report: We present a complex clinical case of a 57-year-old black male patient with moderate hypertension. He presented with podagra attacks in 2003 and was diagnosed with gouty arthritis based on sustained hyperuricemia and recurrent acute inflammatory joint attacks. He has received irregular treatment for years and has been hospitalized several times for severe acute gout attacks, systemic renal and cardiovascular complications, progressing to severe chronic polyarticular tophaceous arthritis.
Conclusions: A clinical, metabolic, histochemical, and reconstructive tomographic imaging diagnosis of uncontrolled chronic pathological osteoarticular damage was performed. We also present a concise epidemiological, etiopathogenic, clinical, and therapeutic update on this important global health problem.
