Catatonia is a neuropsychiatric syndrome characterized by altered movement, and its etiology is multifactorial. This syndrome is associated with psychiatric disorders, such as schizophrenia, and its progression depends on appropriate patient management, as it can evolve into neuroleptic malignant syndrome or even death. We report the case of a patient with clinical condition corresponding to catatonic syndrome, secondary to decompensation of their underlying illness (schizophrenia) due to a septic episode, which progressed to neuroleptic malignant syndrome due to additional complications during hospitalization. Timely diagnosis was made to this patient; he was treated with electroconvulsive therapy in the Intensive Care Unit, achieving a satisfactory response, as it is the treatment of choice in these cases.
