Breast Cancer with a 30-Year History
Abstract
Introduction: Breast cancer is a clinical presentation of cutaneous metastasis from breast cancer, which spreads via lymphatic channels and is classified as classic, based on its frequency of occurrence.
Case report: A 72-year-old woman with a 30-year history of left breast cancer underwent surgical treatment with quadrantectomy and lymph node dissection. In 2019, she presented for the first time with an erythematous pustular lesion on the surgical wound scar. Treatment and evaluation were indicated in the Oncology Department, leading to a total mastectomy. After surgery, the reappearance of the erythematous, papulonodular lesions with purulent discharge and pain on the anterior chest was observed, with no clinical improvement; therefore, a referral to the Dermatology Department was decided. Physical examination of the skin revealed an exophytic swelling of the right breast with multiple ulcerated, necrotic, erythematous-violaceous nodules with irregular borders, infiltrated, malodorous, and a taut, shiny skin surface. Multiple infiltrated, irregular, ulcerated, malodorous, erythematous-violaceous nodular lesions with purulent discharge were identified on the left hemithorax at the level of the previous surgical scar. A skin punch biopsy of the breast revealed a cutaneous metastasis of infiltrating ductal carcinoma of the breast.
Conclusions: Despite early diagnosis programs for breast cancer, cases are still being diagnosed at advanced stages; therefore, it is necessary to consider cutaneous manifestations suspicious for breast carcinoma.
