Psychosis and Coagulation Disorders in Sheehan Syndrome

Abstract

Introduction: Sheehan syndrome is a rare hormonal disorder caused by a lesion in the pituitary gland due to severe postpartum hemorrhage, which can occur in peripartum complications. Typical symptoms include: absence of menstruation, inability to breastfeed, loss of body hair and other symptoms related to hormonal deficits. However, in some cases the diagnosis is often omitted if the symptoms appear sometime after delivery.

Objective: To report a case with Sheehan syndrome.

Case report: A 54-year-old woman with Sheehan syndrome, who experienced an acute psychotic episode, chronic hyponatremia, bicytopenia and prolonged partial thromboplastin time. After a thorough evaluation, the diagnosis of Sheehan syndrome secondary to a postpartum hemorrhage that occurred 20 years ago was confirmed.

Conclusions: The delay in the appearance of psychotic symptoms is highlighted. The initial lesion did not completely affect the pituitary gland, which allowed it to compensate for hormonal deficits. However, the lesion progressed and pituitary insufficiency developed. In addition, unusual extension of the partial thromboplastin time was observed.
