Association of Epidemiological, Clinical, Radiological, Microbiological and Xpert Criteria with the Diagnosis of Active Pulmonary Tuberculosis

Introduction: Tuberculosis is a re-emerging and infectious disease caused by Mycobacterium tuberculosis.
Objective: To determine the level of individual association of the epidemiological, clinical, radiological, microbiological criteria and the Xpert MTB/RIF with the diagnosis of active pulmonary tuberculosis.
Methods: A case-control study was carried out from September 2020 to March 2022 in patients with suspected active pulmonary tuberculosis at the time of admission or during their stay in the Internal Medicine Service at Vladimir Ilich Lenin University General Hospital. The characteristics of the groups were described. The sensitivity and specificity of smear microscopy, culture, and Xpert MTB/RIF testing were determined.
The level of association of the criteria with the diagnosis of active pulmonary tuberculosis was calculated using Cramer's V.
Results: 38.9% of the cases were between 51-60 years old, 61.1% were male and had white skin. The radiological criteria were present in 94.4% of the cases and the Xpert MTB/RIF test was positive in 100% of the patients. Smear microscopy and culture shared 88.89% of sensitivity (95% CI: 71.59%-100%) and 100% of specificity (95% CI: 98.75%-100%). The level of association of the X-pert MTB/RIF test with the diagnosis of active pulmonary tuberculosis was 0.93 (p < 0.01).
Conclusions: Radiological diagnostic criteria, smear microscopy, culture and the Xpert MTB/RIF test presented a strong association with the diagnosis of active pulmonary tuberculosis.
