Pancreatopleural Fistula Secondary to Chronic Pancreatitis

Abstract

Introduction: A pancreatic fistula is considered any abnormal communication between a pancreatic duct and another epithelial surface with the extravasation of fluid rich in pancreatic enzymes.

Objective: To describe the experience in diagnosis and therapeutic management in a patient with non-iatrogenic pancreatic fistula.

Case report: The case of a male patient with history of chronic pancreatitis of alcohol origin is reported. This individual had symptoms of acute respiratory failure, abdominal pain and fever.

Conclusions: Pancreatopleural fistulas are not frequent entities, however they should be suspected in any case of right pleural effusion after manipulation of the pancreatic gland, retroperitoneal invasive procedures and/or history of pancreatitis. This case had favorable resolution with minimally invasive therapy (thoracentesis), use of octreotide and antibiotic therapy.
