Abstract

Introducción: Paraneoplastic pemphigus is an autoimmune blistering disease associated with an occult or previously diagnosed neoplasia, with well-defined clinical, histological and immunological manifestations.

Objective: To present paraneoplastic pemphigus associated with kidney tumor.

Case report: A 47-year-old male patient was reported, with a history of bronchial asthma since childhood, high blood pressure for three years, in addition to having cataract and gastroesophageal reflux due to hiatal hernia, motivating his admission due to erosive lesions of his oral mucosa, painful with hemorrhagic crusts and saliva affecting the cheeks, tongue, gums, as well as lesions in the conjunctival and nasal mucosa. Histopathology revealed acantholysis and necrotic keratinocytes, respectively. The presence of a kidney tumor confirmed the diagnosis. Embolization of the kidney tumor was performed together with high doses of corticosteroids for the cutaneous-mucosal lesions, with good response to said treatment, although the presence of paraneoplastic pemphigus associated with a kidney tumor is rare and should be taken into account among the differential diagnoses of periorificial erosive dermatoses.

Conclusions: It is essential to recognize the importance of the correlation of clinicopathological and immunological findings in paraneoplastic pemphigus, as well as the possibility of presentations that require prompt confirmation and immunopathological evaluation of an occult neoplasia.
