Poliartritis, neuropatía periférica y púrpura palpable en mujer joven

Abstract

La afección del sistema nervioso debida a vasculitis en la artritis reumatoide es una infrecuente manifestación extraarticular y es ocasionada  por isquemia y edema patogénico más que por destrucción axonal o desmielinización. Se presenta una mujer de 26 años con historia de poliartritis autolimitada en el año 2016 por crisis articular intensa, invalidante, bilateral y simétrica de pequeñas y grandes articulaciones asociada a livedo reticularis y un estudio de neuroconducción que reveló mononeuritis múltiple axonal de las extremidades. Se comprobó el diagnóstico de polineuropatía vasculítica secundaria a artritis reumatoide es ésta una causa infrecuente de dolor invalidante y pérdida de fuerza en pacientes con dicha artritis. Su reconocimiento temprano resulta de gran importancia para su correcto tratamiento y mejor pronóstico.
