Fibrilación auricular por vía accesoria oculta, lo que los medicamentos pueden desenmascarar

Las vías accesorias son fibras que comunican la aurícula o el nodo auriculoventricular con el ventrículo, por fuera del sistema normal de conducción del nódulo y del sistema de His-Purkinje. La fibrilación auricular conducida a través de una vía accesoria es una de las taquicardias más temidas, ya que puede desencadenar una fibrilación ventricular y la muerte súbita del paciente. Por tal motivo se recomienda ablación de la vía accesoria en todos los pacientes con fibrilación auricular que conduce a través de ella. Todavía es más difícil el diagnóstico de esta arritmia si la vía accesoria está oculta. Se presenta el caso de un paciente al cual se le diagnosticó taquicardia auricular vs. fibrilación auricular aberrada, al comenzar tratamiento con atenolol se desenmascaró vía accesoria; de forma retrospectiva se realizó el diagnóstico de fibrilación auricular por vía accesoria oculta.
